Nowadays, the surgical treatment for breast cancer is divided into two types. Surgery on the breast, and surgery on the axillae.
Surgery Types For Breast Cancer
Different surgical treatments are performed on the patient’s breast. Some of the procedures we perform in Valencia, Spain are:
Breast-Conserving Treatments (Tumorectomy or Quadrantectomy)
It is the partial removal of the tumor with a margin of healthy tissue around it. The goal is to preserve as much mammary tissue as possible to get the best aesthetic result but ensure the complete excision of the tumor with an optimal safety margin.
In some circumstances, the lesions are not evident to touch so it is necessary to place a metal guide before the surgery aiming at a correct identification of the lesion during surgery.
Mastectomy Surgical Removal
It is the removal of the entire mammary gland, including part of the skin and the areola-nipple complex. This procedure is performed only in selected cases where it is not possible to preserve part of the breast for oncological reasons.
The type of surgery that is decided will depend on the stage of the cancer, the size of the tumor, the size of the breast and the involvement of lymph nodes. Many women undergo breast reconstruction to rebuild the breast after a mastectomy.

Surgical Treatments on the Axillae: Sentinel Lymph Node Technique

- This type of surgery for breast cancer performed on the axillae is the removal of the first lymph node (nodes) of the armpit. This is the sentinel lymph node and it would be the first lymph node to receive metastasis from the tumor.
To do this, a substance (isotopic colloid) is injected around the tumor before the surgery. The histologic study of the lymph node is carried out during the same surgical procedure and if negative, the dissection and removal of the rest of the axillary lymph nodes are avoided, significantly reducing the morbidity associated with a complete axillary lymphadenectomy.

Axillary Lymphadenectomy:
It is a radical surgery on the lymphatic chains of the axillae and includes the removal of all of the axillary fatty tissue. It is indicated for the following cases:
- Sonographic and histologic confirmation (biopsy) of the presence of malignancy in suspicious axillary lymph nodes.
- Sentinel lymph node not identified during surgery.
- Sentinel lymph node with tumoral cells in the intraoperative study.
